The Centers for Medicare & Medicaid Services (CMS) has introduced a new voluntary nationwide initiative, the Guiding an Improved Dementia Experience (GUIDE) Model. This innovative program is designed to enhance the support provided to individuals living with dementia and their unpaid caregivers.
For Medicare beneficiaries diagnosed with dementia and enrolled in Traditional Medicare, the GUIDE program offers additional benefits at no cost to beneficiaries.
In-Home Assessment
We meet with you and your family of choice in your home to work together on understanding dementia and put a plan in place to best support you.
Care Across Settings
Our team will remains available if there are hospital visits, rehabilitation stays, home health, and transitions in living environments. We provide support through these important, and sometimes difficult, transitions in care settings.
Plan of Care for your Doctors
We create a plan together, you and your family lead the choices and we will be sure to provide that plan to your medical team. You are in control of your future; we make sure everyone knows what you want.
24/7 support
A member of our care team is available to help your team navigate next steps outside of traditional business hours.
Respite Care
Caregivers receive up to $2500 annually to receive a much needed caregiving break.
